Supporting Children’s Transitions (blended learning)

Welcome to Supporting Children’s Transitions, one of the core skills modules from our Care Matters course, supporting people’s well-being. This is the blended learning module, with three hours group learning and two hours e-learning, research and assessment. You should already have the date for your group learning. Please contact us if you aren’t sure.

Like all our modules, Supporting Children’s Transitions is person-centred and based on research, policy, practice and the expertise of experience. Successful completion of the course and the on-line final assessment leads to the award of a module certificate. Successful completion of five or more modules leads to the Care Matters course certificate.

Learning Outcomes

By the end of the module, you should be able to:

  1. Identify planned and unplanned transitions experienced by young people

  2. Summarise current legislation and guidance on stability and transitions

  3. Communicate effectively with young people about transitions

  4. Support young people when an unplanned move has been decided.

Module Flow

Your module is divided into 7 units, including an introduction to our learning centre. As a blended learning module, combining group training with e-learning, only the first two and the last unit are completed on-line. The other units form the content of your training session. Copies of presentations from the session will be available here afterwards.

  2. Introduction to module resources and initial assessment
  3. The who, when, what and why of transitions
  4. The transitions legal framework
  5. Communication in transitions
  6. Child-centred practice
  7. Learning review and final assessment.

Module Reading

The module handbook contains all the essential information you need, along with reflective questions to help you learn. You need to bring a copy to your group training session.

The module resources page contains links to research, legal documents, websites and books. These are not required reading, they are resources for your reference, to access when you need them.
